Who we are

Averdeno Health Group Inc. is a private medical services provider incorporated in Canada, with clinical pavilions in Niagara Falls, Toronto, Montréal and Vancouver. This policy explains how we handle personal information, including personal health information.

Information we collect

We collect identity and contact details, information about your stay and travel, the clinical information you provide or that arises from your care, payment details and, where you use this website, basic technical data such as browser type and pages viewed.

Why we collect it

To deliver and coordinate medical care, to schedule appointments and transport, to bill accurately, to meet legal and professional record-keeping obligations, and to improve our services. We do not sell personal information under any circumstances.

Consent

Care is provided on the basis of informed consent. You may withdraw consent for non-essential processing at any time, including marketing communication, without affecting your access to clinical care.

Sharing

Clinical information is shared only with clinicians involved in your care, with third parties you nominate in writing such as your physician at home or your insurer, and where Canadian law compels disclosure. Resort partners never receive clinical information.

Storage and security

Records are held on encrypted servers located in Canada, with access controlled by role, logged and audited. Paper records are stored in secured facilities. Retention is ten years from the last encounter for adults, longer where law requires.

Your rights

You may request access to your record, request correction of factual errors, ask for an account of disclosures and lodge a complaint. Requests are fulfilled within thirty days and normally much sooner.
